Formula feeding

The core of formula feeding is to prepare it exactly as directed, handle it hygienically, and feed by your baby's cues. Formula-fed babies can be raised with just as much closeness and connection.

Preparing it safely

  • Wash your hands and use cleaned, sterilized bottles and equipment.
  • Follow the water-to-powder ratio on the package exactly. Too concentrated risks dehydration and strain on the kidneys; too dilute risks poor nutrition and low sodium.
  • Use only the scoop that comes with the formula, leveled off, not packed down.
  • The WHO recommends preparing powdered formula with water at 70ยฐC (158ยฐF) or hotter, because powdered formula is not sterile and can rarely contain bacteria. Then cool it to body temperature, for example under cold running water.
  • Test a few drops on the inside of your wrist; it should feel lukewarm. Avoid microwaving, which creates hot spots that can scald.
TIPThe right kind of water (boiled tap water, bottled water and so on) can vary by area and by baby. If your baby was premature or has a weak immune system, ask your doctor.

Storage times and hygiene

  • Feed prepared formula as soon as possible, and use it within 2 hours at room temperature (CDC).
  • Once a feed starts, finish it within 1 hour and discard leftovers; saliva can let bacteria grow.
  • Refrigerate formula prepared in advance right away and use it within 24 hours.
  • Take bottles and nipples apart and wash them right after use. Sterilize carefully especially in the early months, for premature babies, and when your baby is ill.
  • Close opened formula tightly, store it somewhere cool and dry, and follow the use-by period after opening printed on the product.

How much to feed

The age chart on the package is only an average. Intake varies from baby to baby and day to day. Newborns take small amounts often; as they grow, each feed gets bigger and feeds get further apart.

Your baby's cues matter more than numbers. Hungry babies move their mouths, suck their hands and root; full babies turn away, push the nipple out and slow their sucking. Do not push them to finish the bottle.

  • Responsive feeding: start and stop by your baby's cues rather than finishing a fixed amount.
  • Paced bottle feeding: hold the bottle closer to horizontal and pause now and then to reduce overfeeding and swallowed air.
  • Never prop a bottle for your baby to feed alone; it raises the risk of choking and ear infections.
  • Do not add cereal to the bottle unless a doctor advises it.

Types of formula and switching

Most babies do well on standard infant formula. Special formulas such as extensively hydrolyzed or amino-acid formulas are for medical reasons like cow's milk protein allergy, used with a doctor's advice. Goat's milk, soy or rice drinks and homemade formula cannot replace infant formula.

Slightly looser or firmer stools are not a reason to switch formulas often. If you switch, discuss why with your doctor and watch how your baby responds for a few days.

Combination feeding and next steps

  • Feeding both breast milk and formula is common and a fine choice. To keep up your milk supply, avoid cutting nursing or pumping sessions too much.
  • Babies who drink enough formula usually do not need separate vitamin D, but it depends on intake, so check at checkups.
  • Even after solids start around 6 months, breast milk or formula remains the main nutrition until the first birthday.
  • Regular cow's milk can become the main drink after 12 months. Before then, do not give it as a main drink.
